On the right is a list of all the forms that will be required for the registration process.  Please make as many copies of each form as needed in order that each camper / sponsor will have the proper paperwork.


  1. A Master Registration List must be filled out for each week that campers are attending.  Simply check the appropriate box on top for the week attending and then list all campers and sponsors coming that week from each church.  Please do not put campers for different weeks on the same list.  Instead, use a different registration list for each week.  Please include this list when camper forms are sent in for registration. 


  1. Camper Registration Package must be completed for each camper upon registration.  This package includes a Note to Parents (information only), a Camper Information Form, and a Medical Information - Wavier Form.  This package must be completely filled out with appropriate signatures in order for each camper to attend.  The PDF file Camper Registration Package includes all the forms needed for each student and can be easily printed off for each student.


  1. Sponsor Application.  Each sponsor, 18 years of age and older, MUST complete the Sponsor Application form.  Also, they should read and be familiar with the Sponsor / Counselor Responsibilities / Camp Procedures / Code of Conduct / Dress Code.  The PDF file Sponsor Application contains all needed forms and information. (Applications due May 15th)


  1. Informational Forms:  All these forms are included in the above packages however they are accessible individually if needed.


  1. Junior Counselors are NO LONGER are available.  (See STARR Leadership Challenge link). If a student in grades 9-12th attempts to register during either Jr High Week or Kids Week without prior approval, they will be asked to leave.
